This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Who We Are
Preamble LLC (“Preamble Health,” “we,” “us”) is a medical practice required by law to maintain the privacy of your protected health information (“PHI”), to provide you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How We May Use and Disclose Your Health Information
Treatment. We use and share your PHI to provide and coordinate your care, including with other providers, laboratories, and pharmacies involved in your treatment.
Payment. We may use and share PHI to bill and collect payment for services, including with payment processors and, where applicable, your health plan or HSA/FSA administrator.
Health care operations. We may use PHI to run our practice, improve care quality, and train staff.
Appointment reminders and care communications. We may contact you about appointments, results, and care plans, including by email or text if you've consented to electronic communications (see our Privacy Policy and Terms).
Business associates. We share PHI with vendors that perform services for us (e.g., secure data hosting) under contracts requiring them to safeguard it.
As required or permitted by law. This includes public health reporting, health oversight, responses to lawful requests, and preventing a serious threat to health or safety.
Uses Requiring Your Written Authorization
We will not use or disclose your PHI for marketing purposes, sell your PHI, or share psychotherapy notes without your written authorization. You may revoke an authorization at any time, in writing, except where we've already acted on it.
Your Rights
You have the right to: inspect and receive a copy of your health record; request corrections to it; request an accounting of certain disclosures; request restrictions on certain uses and disclosures; request confidential communications by alternative means or locations; receive a paper copy of this notice on request; and be notified if a breach compromises the privacy of your PHI. Changes to This Notice
We may change this notice and the changes will apply to information we already hold. The current notice is posted on this page with its effective date, and copies are available at our office.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at privacy@preamblehealth.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
