Does Preamble replace my Primary Care Physician?
A longevity doctor complements your primary care by focusing on prevention, performance, and long-term health optimization — we work alongside your existing primary care team.
Does Preamble have a referral program?

Do I have to live in Arizona to join Preamble?
No, many patients travel to Scottsdale for initial testing and continue with virtual physician sessions and follow-ups.
Are additional testing options available?
We offer a range of advanced testing options including full-body imaging, DEXA scan, bloodwork, strength and movement analysis, cardiac scans, specialty lab panels, and cancer risk screening.
Are consultations in-person or via Telehealth?
Testing occurs in-person at our Scottsdale clinic. Physician consultations and follow-ups are available via secure telehealth for your convenience.
Is any wearable technology or apps used or required?
Yes. We often use continuous glucose monitors (CGMs) and integrate data from wearables like Oura and Apple Health to give you a fuller picture of your health.
Pricing
Do you take health insurance?
Preamble does not accept insurance. This allows us to provide fully personalized, unrestricted care without the limitations of insurance-driven protocols.
Can I pay with my HSA or FSA account?
Yes, many of our services are eligible for HSA/FSA reimbursement. We recommend confirming with your plan administrator.
Can I pay for the Program or testing on a monthly or quarterly basis?
Yes, flexible billing options including monthly and quarterly plans are available for our membership programs.
What does the annual Program cost?
Our Medicine 3.0 Executive Physical is $2,750. Annual Memberships start at $650 for our Core Membership. Our Essentials Membership is $2,000/year and our Pro Membership is $4,000/year.
Do you offer a discount for retired or active duty military personnel?
Yes, discounts are available for retired and active duty military personnel. Please mention your service status when booking.
